Venita Middlebrook Is the best thing that ever happened to advanced care group. She takes care of the clients and always has their best interest at heart. She definitely cares about the clients and it shows. This is the place for palliative care!!

I work closely with The Advanced Care Group as a healthcare partner. Their team demonstrates compassion, professionalism, and genuine commitment to patient-centered care. Their office staff and field clinicians offer emotional and practical support, and I would confidently recommend them to any family who could benefit from palliative care.

My brother received care from ACG and then had to go to hospice. Due to a series of events we needed ACG to help us with a referral to additional treatment at a later date. His NP Patty Clark and the staff went above and beyond to help us get him the care he needed even though there was no benefit to them at all. They were a huge help and we appreciated it.

The Nashville area team at Advanced Care Group is fantastic! They are attentive to the needs to the patients, families and collaborative partners. They respond timely and are solution oriented. I enjoy working with them.

My Mom began receiving care from Advanced care Group in March of 2020 after she was discharged from a short-term rehab after spending 6 months in a cancer treatment center in Nashville. I was relieved that they offered NP that came to you as she was not able to walk yet after 6 rounds of chemo at 77 years old. During 2020 we had a new NP almost each month but by the end of that year we had one provider who was great. The phone staff went through a lot changes and getting through to a NP was always a challenge, relaying messages was just as challenging. Last year the providers changed twice and this year twice, however, the care remain consistent. Suddenly a new NP was assigned, she came in and spoke with my Mom and we went through all of her medications ensuring she had the info to refill all of her medications she had been prescribed since 2019 while in cancer treatment. She asked if we had an appointment with a behavioral specialist, as they no longer preferred to prescribe those meds. Unfortunately, due to having a bad UTI she missed the appointment we had. I called and was told for an entire week that the NP would refill one of those meds. As the week went on it changed from 'she will' to 'we will have the NP call you". When she did, she was rude and would not let me speak. She incorrectly informed me that my Mom only gets one of those medications on a PRN basis. Not true as she rec'd that twice a day for years. As she continued to argue with me and speak over me I asked that she let me speak and yes my tone was not a quiet one. She had a telehealth appt with my Mom and wrote the medication. The following week she was out of her anxiety medication and can go into seizures without it. I rescheduled her psych appt and gave them that date and that I would ensure she made it to that appointment. Same situation. I called for 5 days straight and on that Friday (Day 5) she did not call me back but called in a 2 week supply. Today I received a letter discharging her from the Practice because she did not follow instructions and I was inappropriate during the phone call. I would have preferred a letter stating that as of X date they would no longer prescribed meds, A, B, and C. I would have ensured we had another provider for those meds and that she would make that appointment even if she was sick. This type of care is to make care easier on those who suffer from long-term illness. Imagine if my Mom had to advocate for herself. Long story short, they want to avoid liability and no longer want to prescribe meds that unfortunately many people still need, and as usual the ones who abuse such medications give those that truly need them a bad reputation.

I am so very happy with Advanced Care Group! Since my car wreck in 2019 that left me paralyzed from the chest down, Patty Clark has been such a God-send! She is always here when she says she will be, always fulfilling my needs, and even listens to me whine about my personal life. Patty has become a staple in my life, medically and therapeutically. I couldn’t imagine being homebound, as I am, without her! Thank you ACG for sending her Patty Clark my way! She is AWESOME!!

This place deserves 0 stars. Great concept...poor execution and if you're unlucky enough to have Janet Mooney as your NP - GOOD LUCK. She is extremely poor with follow-up. She will have to be contacted multiple times. She provides the same medical information that the news stations provide. She has contradicted the CDC and local health departments. I have had multiple conversations with the medical director, he doesn't care. If you need pallative care DO NOT use Advanced Care Group.

My Dad has been referred to them for palliative care through the case manager at the hospital over the weekend. Honestly knew nothing of the reviews of the group but wished I had beforehand. The nurse practitioner came yesterday to do the initial patient evaluation and started him on a plan. That part went okay and the NP was very kind and helpful. Some of the medication adjustments made left him in agonizing pain overnight so first thing this morning my mom called and left a message with the answering service to have the NP call us back so we could talk more about making more changes. They said they would pass along message. By lunch we heard nothing so she called back. Answering service told her everyone was at lunch and couldn’t talk and to call back at 1pm. Mom called at 1:10pm and the answering service representative (she never said her name) said no one was available at this time and HUNG UP on my mother. I am appalled with this customer service and the way they have treated my mother today. Given the terminal illness my father has (and all other patients within a palliative care group) true care from everyone should be given. After this, considerations will be given to a different group who’s better willing to assist.

We have an awesome nurse practitioner in Jennifer Weaver. My husband is going through chemo and she has been out every week to check on him. She's very professional but also very personable. She has been very helpful in making sure he is comfortable and has what he needs. I would recommend this company to anyone
